NYS Required School Health Form / PHYSICAL EXAM
NYSED has mandated health appraisal forms (physicals) for our students be on file at the health office.
- Kindergarten
- 1st Grade
- 3rd Grade
- 5th Grade
- 7th Grade
- 9th Grade
- 11th Grade
- All students who are new to the District (regardless of incoming grade level)
Physicals are valid for one year: Any physical performed by a New York State physician on or after September 6, 2023 will be considered current.
Screenings
NYSED requires regular health screenings. According to NYSED:
School health screenings help in the early detection of health conditions and help children lead a healthy life. Among other things, school health screenings can help detect vision, hearing, and identify spinal deformities that may cause impairment of the body's range of motion and endurance.
To ensure that all children and youth receive these important screenings, public schools are required to conduct student health screenings: vision, hearing, and scoliosis in the mandated grades if they are not documented on a health exam.
If the state requests screenings, your child will have a grade level screening as listed below if it is not listed on your child's physical. A notification will be sent home if there are any findings that would cause concern or need medical follow-up. The following screenings are required:
- Vision and Hearing Screenings: Grades K, 1, 3, 5, 7 and 11 (details in link below)
- Scoliosis Screening: Girls in Grades 5 and 7; Boys in Grade 9
- Distance Vision, Near Vision, Color Perception, and Hearing Screenings: All new students transferring into the district.
